The Defense Department inspector general this month is expected to release its independent review of an Air Force investigation into the November 2010 fatal F-22 crash in Alaska.
The review was announced last year, with the DoD IG sending a letter to Air Force Secretary Michael Donley saying it will investigate the Air Force's Accident Investigation Board report on the crash to "verify that AIB conclusions are supported by evidence of record consistent with standards of proof."
Capt. Jeff Haney was killed in the Nov. 16, 2010, crash near Joint Base Elmendorf-Richardson. Air Force AIB president Brig. Gen. James Browne wrote in the report that the "cause of the mishap was the MP's [mishap pilot] failure to recognize and initiate a timely dive recovery due to channelized attention, breakdown of visual scan and unrecognized spatial disorientation."
The report found that a malfunction of the jet's bleed air intakes caused an automatic shutdown of multiple aircraft systems, including the jet's primary oxygen system.
Air Force leaders later seemed to walk back the blame on the pilot, with former Chief of Staff Gen. Norton Schwartz telling Congress last March that the Air Force "did not assign blame to the pilot … This was a complex contingency that he did his best to manage and, in the end, he lost aircraft control."
Last August, Haney's widow, Anna Haney, settled a lawsuit with makers of the jet, in which she alleged the jets are dangerous and defective and the contractors were at fault in the crash.
